Supervisor certificate approvals
All forms were updated 8 March 2012 with some tminor ext changes and address corrections. The new versions also have a smaller masthead logo to overcome a design issue.
CS01 Application for supervisor certificate PDF
A person who holds a supervisor certificate may be appointed as the nominated supervisor of an education and care service, or may consent to be placed in day-to-day charge of a service in the absence of the approved provider or the nominated supervisor.
CS02 Request for supervisor certificate for specified class of persons PDF
Certain classes of persons may be automatically granted a supervisor certificate without having to apply. For further information about this type of supervisor certificate see the Guide to the National Law and National Regulations.
CS03 Application to amend a supervisor certificate PDF
A certified supervisor may apply to amend their supervisor certificate at any time.
CS04 Application for voluntary suspension or surrender of a supervisor certificate by certified supervisor PDF
A certified supervisor may apply to suspend their supervisor certificate for up to 12 months. A certified supervisor may also surrender their supervisor certificate.
CS05 Notification of change of circumstances of a certified supervisor PDF
An approved provider must notify the regulatory authority of certain circumstances and information as specified in the Regulations. For more information refer to the Guide to the National Law and National Regulations.
PA02 Declaration of fitness and propriety PDF
Compulsory for individual provider applicants, persons with management or control of a service and certified supervisor applicants.
This form is to be completed by an individual provider applicant and in the case of an entity provider applicant, all persons with management or control of a service.
Note: NSW applicants must also complete a Working with children check - applicant declaration and consent form.
AR01 Application for internal review of reviewable decision PDF
