Service approvals

All forms were updated 8 March 2012 with some minor text changes and address corrections. The new versions also have a smaller masthead logo to overcome a design issue.
SI01 Notification of serious incident   PDF

An approved provider must notify the regulatory authority of any serious incident meaning an incident or class of incidents prescribed by the National Regulations as a serious incident. For more information refer to the Guide to the National Law and National Regulations.

NL01 Notification of complaints and incidents (other than serious incidents)   PDF 

An approved provider must notify the regulatory authority of any complaints or incidents at the approved education and care service operated by the approved provider. For more information refer to the Guide to the National Law and National Regulations.

SA01 Application for service approval – Centre-based care   PDF

An approved provider may apply to the regulatory authority for a service approval if they will be the operator of the service and will be responsible for the management of the staff members and nominated supervisor of that service. This form should be accompanied by form NS01.

SA02 Application for service approval – Family day care   PDF

An approved provider may apply to the regulatory authority for a service approval if they will be the operator of the service and will be responsible for the management of the staff members and nominated supervisor of that service. This form should be accompanied by form NS01.

SA03 Application for amendment of service approval   PDF

An approved provider may apply to amend their service approval at any time.

SA04 Notification of transfer of service approval – Centre-based   PDF

A service approval may be transferred from one approved provider to another approved provider – for example, where an approved provider sells their business, or has their provider approval or service approval suspended. The transferring and receiving approved providers must jointly notify the regulatory authority.

SA05 Notification of transfer of service approval – Family day care PDF

A service approval may be transferred from one approved provider to another approved provider – for example, where an approved provider sells their business, or has their provider approval or service approval suspended. SA06 Application for voluntary suspension of service approval  PDF

An approved provider may apply to suspend a service approval for up to 12 months.

SA07 Notification of surrender of service approval  PDF

An approved provider may surrender a service approval.

SA08 Application for service or temporary waiver  PDF

The National Law provides for two types of exemptions or waivers from meeting a prescribed element or elements of the National Quality Standard or the National Regulations:

  • service waiver – on an ongoing basis
  • temporary waiver – for a period of up to 12 months

SA09 Application for revocation of service waiver  PDF

An approved provider may apply to revoke their service waiver at any time.

SA10 Application for re-assessment and re-rating (service rating)   PDF

An approved provider may apply to the regulatory authority for a reassessment and re-rating. The National Law permits an application for reassessment to be made once every two years.

SA11 Application for review of ratings by regulatory authority (service rating)  PDF

An approved provider may request the regulatory authority to review a rating determined for an approved service. For more information refer to the Guide to the National Law and National Regulations.

SA12 Notification of change of information about an approved service  PDF

An approved provider must notify the regulatory authority of certain circumstances and information as specified in the Regulations. For more information refer to the Guide to the National Law and National Regulations.

NS01 Nominated supervisor consent form  PDF

The approved provider must designate a nominated supervisor for each service they operate or intend to operate and the nominated supervisor must give their written consent. (This one page form must be included with an application for service approval, SA01 or SA02, and a notification of change to nominated supervisor, NS02).

NS02 Notification of change to nominated supervisor  PDF

The approved provider must notify the regulatory authority if the provider wishes to change the person designated as the nominated supervisor. This form should be accompanied by form NS01.

AR01 Application for internal review of reviewable decision  PDF

A person who is subject to a decision, as prescribed in the Regulations, may apply to the regulatory authority for a review of the decision. For further information about the types of reviewable decisions see the Guide to the National Law and National Regulations.
