Provider approvals
All forms were updated 8 March 2012 with some minor text changes and address corrections. The new versions also have a smaller masthead logo to overcome a design issue.
PA01 Application for provider approval PDF
A provider approval authorises a person to apply for one or more service approvals.
PA02 Declaration of fitness and propriety PDF
Compulsory for individual provider applicants, persons with management or control of a service and certified supervisor applicants.
This form is to be completed by an individual provider applicant and in the case of an entity provider applicant, all persons with management or control of a service.
Note: NSW applicants must also complete a Working with children check - applicant declaration and consent form.
PA03 Application for amendment of provider approval PDF
An approved provider can apply to amend their provider approval at any time.
PA04 Application for voluntary suspension of provider approval PDF
An approved provider may apply to suspend their provider approval for up to 12 months.
PA05 Notification of surrender of provider approval PDF
An approved provider may surrender a provider approval. Where this occurs, all service approvals held by the provider are also cancelled or taken to be surrendered.
PA06 Application for provider approval by executor of estate of approved provider PDF
Where an approved provider can no longer fulfil their role due to death, a nominated executor can be appointed, subject to meeting the requirements to be an approved provider.
PA07 Application for provider approval by legal personal representative or guardian on incapacity of approved provider PDF
Where an approved provider can no longer fulfil their role due to incapacity, a nominated legal personal representative or guardian can be appointed, subject to meeting the requirements to be an approved provider.
PA08 Notification of change to information about approved provider PDF
An approved provider must notify the regulatory authority of certain circumstances and information as specified in the Regulations. For more information refer to the Guide to the National Law and National Regulations.
AR01 Application for internal review of reviewable decision PDF
A person who is subject to a decision, as prescribed in the Regulations, may apply to the regulatory authority for a review of the decision. For further information about the types of reviewable decisions see the Guide to the National Law and National Regulations.
