Application Forms
From this page you will find forms that may be required under the new arrangements in place from 2012. Regulatory authorities in each jurisdiction are responsible for assessing most applications and notifications. Each form contains the contact details for the authority in your state or territory.
All forms were updated 8 March 2012 with some text changes and address corrections. The new versions also have a smaller masthead logo to overcome a design issue.
Western Australian readers, while encouraged to become familiar with NQF arrangements, are not yet subject to the National Law and Regulations and should not submit these forms. Western Australian children's education and care service operators and educators should refer to their regulatory authority, the Department for Communities, for the latest advice and current Western Australian forms.
There are four broad areas that application forms to regulatory authorities fall under:
- Provider approval – these forms should be used by those who own or operate services and want to apply to become an approved provider or amend existing provider status
- Service approval – these forms should be used by approved providers who want to apply to operate a service or centre. Nominated supervisor forms, waiver forms and other forms related to the running of a service can also be found here
- Supervisor certificate approval – these forms should be used by those who want to apply to become a certified supervisor or amend their certified supervisor status.
- Notification forms - these forms should be used by providers and services to notify the regulatory authority. The forms relate to incidents, complaints, changes to information.
Many of the forms require a Provider Approval Number and/or a Service Approval Number. With transitional arrangements currently in place, the regulatory authorities will accept forms with this section left blank.
ACECQA will assess applications from:
- Individuals applying for their qualifications to be assessed
- Organisations applying for their programs to be approved
- Services applying for further review by the Ratings Review Panel
- Services applying for the Excellent rating

Lodging your application
Print and fill in the forms relevant to you. Once you have completed the form you should mail it to your regulatory authority. The addresses and contact information appear near the end of each form under the heading Lodging Your Application.
